Therapists at the Psyched Group accept most commercial health plans. If your insurance does not cover we offer sliding scale Self-Pay rates from $110 – $195/session.
Health Insurance
1. Your insurance policy is a contract between you and your insurance company or your
employer and your insurance company. It is in your best interest to be aware of your outpatient
mental health benefits before your first appointment.
2. If you provide us with your insurance information, we will contact your insurance company prior to your first appointment in an attempt to verify your insurance benefits.
3. As a courtesy to you, we will send you a quote of benefits via email prior to your first appointment so that you are aware of what you can expect to pay each session. Please note: this information should be considered an estimate and not an exact determination.
4. Our office will submit claims to your insurance company per the information you provide.
5. Some insurance companies arbitrarily select certain services they will not cover.
6. It is the client’s responsibility to negotiate with the insurance company for any unpaid
services.
7. You will be responsible for any expenses incurred if your insurance company does not pay
our company what you expected they would.
8. It is your responsibility to inform us when your insurance coverage changes and to provide us
with the appropriate plan information.
9. If there is a lapse in your insurance coverage during the time you are receiving services, you
will be responsible for the full fee of the appointment(s).
10. If your therapist is out-of-network with your insurance and you intend to pay out of pocket for
sessions, we will provide you with a superbill that you can submit to your insurance company. You can use this superbill to seek partial or full reimbursement from your insurance company.
PRIVATE PAY
If you are paying for sessions privately (not through insurance), our private pay fees are as
follows:
1. The private pay cost per session for individual therapy is $110 ($125 for the initial session).
2. Waiver of insurance form is required.
PAYMENT AUTHORIZATION & FINANCIAL CONSENT
Clients must maintain a current credit/debit card on secure file with us through your Secure Portal Account. Clients must also electronically sign a Payment Consent Form for the current credit/debit card on file. Appointments must be suspended if payment cannot be collected by the Psyched Group.
CANCELLATION POLICY
If you do not show for appointment or cancels within 24 hours of the appointment then a $75 “Late Cancel/No Show” Fee is charged. Appointments cancelled before 24 hours of appointment will not be charged because your therapist is able to schedule another client in need. Your therapist may submit a waiver to reduce this fee in cases of hardship.
PRIVACY
We are committed to respecting and protecting your privacy and the confidentiality of your health care information. Protecting your privacy and the confidentially of your medical information is a critical factor in providing mental health services. The code of ethics HIPAA (Health Insurance Portability and Accountability Act) as well as CHAPTER 42 CFR Part 2 (Confidentiality of Alcohol and Drug Abuse Patient Records) mandates that all information about you be protected and that any disclosure of your protected Health Information (PHI) requires your written consent.
